About Us
At Apex Health Alliance, we believe that stable housing is the foundation for a healthy, independent life. Our mission is to support individuals in securing and maintaining safe, long-term housing by providing comprehensive Housing Stabilization Services (HSS). We are dedicated to serving individuals on Medical Assistance, ensuring they receive the resources and guidance necessary to navigate the challenges of housing insecurity.
Our Services
Housing Stabilization Services
Housing Stabilization Services (HSS) is a new benefit for individuals on Medical Assistance to help find and maintain housing.
Housing Stabilization Services has three parts:
- Housing Consultation
- Housing Transition
- Housing Sustaining
Housing Consultation
Assists eligible individuals who do not have access to a case manager or targeted case manager in developing a person-centered plan and submitting it to a Housing Transition provider. Services that assist a person in developing a housing-focused, person-centered plan, assisting the person in obtaining the right state plan resources that support housing stability, and providing referrals or information about other services
